John is uncontrollable, incorrigible, will not attend school. (signed in German; last name written Doll with an umlaut over the "o"). J.R. Duncan & A.H. Dice, Truant Officers for the district where said John Doll resides, depose that the parents would send John off to school and would not see him until 9 or 10 o'clock at night. Additional Comments: Notes made upon the boy's arrival at Pennsylvania Reform School at Morganza: John Dill, No. 5662, Division A. Parents living at 76 Juniata Street, Allegheny, Pa. Father, Henry Dill, a laborer. Sent here for not going to school. Description: height 4' 2 1/2"; weight 62 pounds. Light brown hair & eyes; fair complexion. Age: 11 on Mar. 10, 1899; received Mar. 15, 1899. Born Penna. of German parents. Lutheran. Parents not intemperate, own no property. 1 older sister. Can read & write.
